Dear Patients and Parents/Guardians:
Please be advised that my medical practice will close permanently on August 12, 2022 as I transition into my next career. I will not be able to attend to your medical needs after that time.
You should choose another Allergy & Immunology doctor to be your Allergy provider soon. I suggest that you view the South Texas Independent Allergists website at southtexasindependentallergists.com, the Find an Allergist link on the American College of Allergy, Asthma, and Immunology website at acaai.org, or call your insurance provider to to assist you in choosing your new Allergist.
Your medical records are confidential. Therefore, I will need you to fill out and return the enclosed authorization form to my office via fax at 281.452.3301 or email to wellsallergystaff@gmail.com or in person so I can transfer copies of your medical record to your new physician. You may also find the Authorization to Release Medical Information on our website at wellsallergyclinic.com.
It is very important that you sign the authorization form; otherwise, your new doctor will not have your medical history.
Thank you for your loyalty. I am sorry I cannot continue as your physician at this time. I wish you the best of health in the future. Please contact our office at 281.452.3300 with any questions.
